What is the Tobacco User Premium and how do I avoid it?
Frontier medical plans include a Tobacco User Premium – an additional $50 per month – if you or your covered dependent(s) are tobacco users. Individuals who use tobacco products at least once a week are considered tobacco users.
To qualify for non-tobacco user status and avoid the additional $50 per month, you and your covered dependents must not use tobacco products or must enroll in either the Frontier sponsored “Tobacco Free” program or another bona fide tobacco cessation program or activity prior to Annual Benefits Enrollment.
Each new plan year, you are defaulted to tobacco user status. During Annual Benefits Enrollment, you must identify the tobacco user status of you and your dependents. If you do not, you will default to tobacco user status (even if you identified as non-tobacco user(s) for the prior year) and you will be charged the tobacco user premium.
The 2017 Annual Benefits Enrollment period is November 2 - November 18, 2016. You may enroll online at www.frontierbenefitscenter.com or by phone at 1-855-FTR-2887. Elections made to benefits during this time become effective January 1, 2017.
If you or a covered dependent are a tobacco user, you can avoid the Tobacco User Premium in 2017 by enrolling in either the Frontier sponsored “Tobacco Free” program or another bona fide tobacco cessation program or activity by November 1, 2016. You are required to complete the tobacco cessation program you choose.
To enroll in the Frontier sponsored “Tobacco Free” program, go to www.myhealthylifestyles.com and select “Sign Up.” Enter your name, date of birth, Zip Code and gender and follow the prompts to complete your registration. As an alternative, call 1-877-252-8410 to enroll.
Other bona fide tobacco cessation programs and activities include (but are not limited to): those found at the West Virginia DHHR Division of Tobacco Prevention website www.dhhr.wv.gov/wvdtp/cessation/Pages/default.aspx; the American Lung Association’s Freedom From Smoking program at http://www.lung.org/stop-smoking/join-freedom-from-smoking/; Healthway’s Quitnet at www.quitnet.com; nicotine replacement therapy; and tobacco cessation medications. Employees and/or dependents that choose other programs or activities will be responsible for any associated cost.
